I hereby consent to RESCUEFIT UK LTD capturing, storing and using images of myself (or my dependants) caught in video recordings, and/or photographs, taken/recorded/live streamed. At no point will these be secretly recorded, instructors and/or staff will continue to inform participants of these being taken.
The media will only be used for purposes as described by RescueFit when you complete this form. Please ask for further clarification if required.
I understand that:
• images of me (or my dependants) will be held in accordance with UK Data Protection legislation;
• that the images/video of me (or my dependants) or captured in the video recordings and/or photographs will be the subject of copyright by RESCUEFIT UK LTD & any other intellectual property which arises in the recordings will also belong to RESCUEFIT UK LTD;
• I can ask RESCUEFIT UK LTD to stop using images/videos of me or my dependant at any time, in which case they will not be used in future publications but may continue to appear in publications already printed/posted and in circulation.
